Job Summary

Provide biopsychosocial assessments, counselling, and case management to support residents’ rehabilitation and reintegration at Jamiyah Singapore’s halfway house.

Responsibilities

  • Conduct biopsychosocial assessments and develop Individual Service Plans (ISPs) to address residents’ needs
  • Deliver counselling and crisis intervention services to residents and their families to support emotional and social well-being
  • Prepare accurate and timely case notes, progress reports, and submissions in compliance with organisational and MSF standards
  • Provide Information & Referral (I&R) services to connect residents and families with relevant community resources
  • Plan, implement, and evaluate programmes and activities that promote rehabilitation and reintegration outcomes
  • Advocate for residents’ needs and rights during case conferences and multidisciplinary team discussions
  • Engage families to strengthen support systems facilitating residents’ successful reintegration
  • Participate actively in training, supervision, and reflective practice sessions to enhance professional skills
  • Share knowledge and best practices with team members to continuously improve service quality
  • Uphold professional ethics, confidentiality, and service standards in all client and team interactions
  • Perform additional duties as assigned by the Senior Social Worker or Head of Halfway House

Required competencies and certifications

  • Degree in Social Work or related discipline
  • Registration as a Social Worker with SASW

Preferred competencies and qualifications

  • 2–3 years of casework or counselling experience; strong practicum experience considered for fresh graduates
  • Experience in rehabilitation, community, or residential care settings
  • Ability to work effectively with diverse populations, including high-risk clients and families
  • Proficiency in Microsoft Office and case documentation systems
