Referral Management Coordinator

About the Position: This position is located at Ireland Army Health Clinic on Fort Knox, KY. Serves as a Referral Management Coordinator in the Managed Care Division, Referral Coordination Center. Working hours are from Monday-Friday, 7:30 a.m. - 4:00 p.m. This is a Direct Hire Solicitation

Major duties

• Educate staff, healthcare professionals and beneficiaries on the consult tracking procedures
• Maintain suspense database for contacting civilian healthcare professionals to acquire consult results
• Work closely with network providers to ensure contact information and required chart notes are sent and received in a timely manner to provide quality clinic visits.
• Independently, identify and notify staff of items requiring urgent attention and processing to ensure best healthcare outcomes.
• Maintain positive and clear communication with nurse case managers, group practice managers, providers, and the clinical support division to ensure the patients needs and hospital requirements are followed throughout the process.

Specialized Experience: One year of specialized experience which include: reviewing referrals for completeness, accuracy, and compliance; processing and coordinating referrals; tracking referrals and consult results; maintaining referral or suspense database; using Electronic Health Record (EHR) database or similar program to retrieve, upload, route and document referral information. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-06).
