Responsibilities:
- Case Management
- Review individual care plan with relevant stakeholders
- Participate in case management discussions with MSF.
- Support Therapeutic Team in referral and admission process, including assisting in intake briefings for new admissions
- Assist in duties such as, accompanying residents to the clinic or medical appointments, home visits, prospective employer and build relationship with
- Together with the team, respond to crises — self-harm, cutting, suicidality, acute dysregulation, aggression — with calm and sound clinical judgement
- Administration – assist the Manager:
- Maintain accurate, timely case records and progress records of residents in line with audit and compliance requirements by MSF
- Prepare and vet reports for timely submission to MSF and the Court, and social reports for referrals to a high standard
- Maintain regular communication with volunteers and partnering agencies to ensure clear communication of programme processes.
- Assist with data entry e.g. input and/or update resident information where needed
- Undertake any other duties as directed by the Senior/Assistant Senior Social Worker and/or Head of Centre
Qualifications, Competencies & Experience:
- Degree or Diploma in social work
- Healthy boundary management
- Has an interest in working with trauma
- Knowledge and application of the SASW code of professional ethics and Code of Social Work Practice
- Willing to be supervised and open to feedback for change
- Strong communication skills (written and spoken)
- Team player
- Proficient with MS applications
