Job Summary

We are seeking warm and razor-smart Psychiatric Nurse Practitioners with entrepreneurial spirits to provide psychiatric consultations in nursing homes. This is a travel-based role that combines in-person visits at assigned nursing facilities with telehealth services. Candidates must be comfortable traveling throughout Vermont to provide in-person patient care while also conducting consultations and follow-ups via telehealth.

As a Psychiatric Nurse Practitioner, you will be provided with full administrative support and clinical mentorship. Responsibilities

Travel to assigned nursing homes throughout Vermont to provide in-person psychiatric consultations and patient care.

Conduct psychiatric consultations and follow-up appointments through both in-person visits and telehealth.

Work closely and collaboratively with nursing home staff, residents, families, and other members of the care team.

Proactively follow up with patients and assess the efficacy of recommended interventions.

Provide education to patients, families, and facility staff on psychiatric best practices.

Round at assigned nursing homes and provide psychiatric medication and treatment recommendations.

Integrate pharmacological and behavioral interventions, emphasizing non-pharmacological treatments whenever possible.

Maintain timely, accurate, and thorough clinical documentation.

Requirements

Master of Science in Nursing (MSN)

Active Psychiatric Nurse Practitioner License

ANNC Board Certification

Ability and willingness to travel throughout Minnesota for in-person nursing home visits

Comfortable providing care through both in-person and telehealth modalities

Excellent communication and documentation abilities

Strong patient assessment skills

Strong, ambitious, patient-centric work ethic

Prolonged sitting. Computer work including keyboard and mouse use for extended periods. Lifting/moving patients.
