Care Coordinator - Nottingham

Location: Nottingham Job type: Permanent Salary: £28-35k PA

MD2 Recruitment is working on behalf of a care provider seeking a Care Coordinator to help deliver a reliable, person-centred service in Nottingham. This is an opportunity for someone who enjoys the operational side of care and is equally motivated to grow the service . The successful candidate will coordinate care packages, support care staff and build strong relationships with clients and their families. Business development is a substantial part of the role: you will identify opportunities, develop referral relationships and help the provider increase the number of people it supports.

Key responsibilities

• Coordinate care visits and maintain effective staff rotas, responding promptly to changes and gaps in cover.
• Act as a point of contact for clients, families, care staff and other professionals.
• Support the smooth start of new care packages and help maintain a high standard of service.
• Build relationships with local referral partners and identify opportunities to grow the service.
• Follow up enquiries, meet prospective clients and contribute to converting enquiries into new care packages.
• Work with management to develop and deliver a practical business development plan.
• Support the recruitment, onboarding and retention of care staff as the service grows.
• Maintain accurate records and work in line with safeguarding, confidentiality and regulatory requirements.

About you

You will have experience in care coordination or a similar role within a care service, with strong organisational skills and a calm approach to changing priorities. You will be confident speaking with clients, families and professional contacts, and comfortable taking an active role in developing new business. You should understand the importance of matching care staff to clients, maintaining continuity of care and growing a service without compromising quality.
