We invite you to join our team as: Staff Nurse 5 days work week (Senior Care Centre)

The Registered Nurse is responsible for the overall nursing care, health monitoring and Clinical safety of all seniors attending the centre. The Registered Nurse plans, delivers and Evaluates person-centred nursing care, supervises care staff in health-related tasks, and works closely with families and community partners to support seniors to age well in the community. Job Description Conduct nursing assessments for new and existing seniors (vital signs, cognition, falls risk, nutrition, skin integrity, continence, pain, functional status). Develop, implement and review individualized care plans in consultation with seniors, families and the care team. Monitor seniors' health conditions(e.g. diabetes, hypertension, stroke, dementia, heart disease) and track changes over time. Perform nursing procedures within scope, including but not limited to: wound care and dressing, capillary blood glucose monitoring, injections (e.g. insulin), nasogastric tube care and feeding, urinary catheter care, and stoma care. Act as first responder in medical emergencies (e.g. falls, chest pain, hypoglycaemia, choking, seizures, cardiac arrest), providing BCLS/CPR and AED as needed. Make timely, independent decisions on whether a senior needs doctor review, hospital transfer or emergency services. Report, investigate and follow up on incidents (falls, medication errors, injuries), and recommend preventive measures Guide, supervise and assess care staff (e.g. care associates, healthcare assistants) on delegated health-related tasks. Support centre programmes and all activities (including ICCP)where appropriate. Perform any other related duties as assigned by the Centre Manager. Job Requirements Minimum Diploma in Nursing or Degree in Nursing with a Post Basic Certificate. Must be registered with Singapore Nursing Board.  Minimum1–3 years of clinical experience, preferably in acute hospital, community hospital, nursing home, home care or eldercare settings. Has multiple capabilities, which include personal integrity and self-awareness; ability to formulate and share vision; capability to set goals, plan and implement change; ability to create an atmosphere that supports communication and promotes accountability; and at the same time engender mutual respect among caregivers.
