Job Responsibilities: Conduct intake assessments and participate in case conferences with multidisciplinary teams. Collaborate with healthcare professionals to develop and review Individual Care Plans (ICPs). Monitor clients’ progress and evaluate care outcomes. Conduct home and hospital visits when required. Provide guidance on relevant financial assistance schemes and community resources. Perform other duties as assigned by the reporting manager.

Requirements: Degree or Graduate Diploma in Social Work. Relevant experience in healthcare or community settings will be an advantage.
