Registered Nurse (Home Care) | Up to $5,000 | Perm | West

Roles & Responsibilities 💰 Salary: Up to $5,000/month 🕘 Employment Type: Permanent 📍 Location: Bukit Batok 🌟 Role Overview We are seeking a qualified and experienced Registered Nurse to provide personalised, high-quality home nursing care to patients in the community. This role is suitable for nurses passionate about geriatric care, supporting homebound seniors and delivering compassionate end-of-life care while helping patients maintain their comfort, dignity and quality of life. 🧑‍⚕️ Key Responsibilities Provide specialised home-based nursing care and clinical interventions according to individual patient needs.

Conduct initial nursing assessments and six-monthly reviews, developing and implementing personalised care plans.

Educate and train caregivers on nursing procedures, patient safety and infection control practices.

Identify potential health risks, exercise sound clinical judgement and advise patients and caregivers on appropriate preventive measures.

Maintain accurate and timely clinical documentation and coordinate home visits according to patients’ care needs.

Collaborate with multidisciplinary healthcare professionals to ensure coordinated and continuous patient care.

✅ Job Requirements Registered with the Singapore Nursing Board (SNB) with a valid practising certificate.

Minimum 4 years of ongoing nursing experience in medical or geriatric nursing within the past 2 years, in an acute or community care setting.

Experience in oncology or palliative care will be an advantage.

Good communication and interpersonal skills, with the ability to engage patients, caregivers and healthcare professionals from diverse backgrounds.

Able to work independently, exercise sound clinical judgement and adapt to home-based care environments.

Genuine interest in supporting homebound seniors and providing end-of-life care.

Willing to provide after-office-hours tele-consultation support for end-of-life patients on a rotational basis.

Physically fit to carry work equipment and travel within the community for home visits.
