We are seeking a compassionate, organized, and team-oriented Full-Time Medical Social Worker (MSW) to serve patients in Walnut Creek and surrounding communities.

Responsibilities

Conduct psychosocial assessments during patient admission

Evaluate social, emotional, and environmental factors impacting care

Collaborate on interdisciplinary care plans

Provide counseling, resource linkage, and discharge planning support

Consult with physicians and care teams regarding patient needs and care adjustments

Maintain timely and accurate clinical documentation

Supervise Social Work Assistants as needed

Requirements

Current California Licensed Clinical Social Worker (LCSW) license

Valid California Driver’s License

Reliable transportation for home visits

CPR and First Aid certification

Ability to travel throughout Walnut Creek and surrounding East Bay areas
