Here at Greater Manchester Mental Health NHS Foundation Trust , we're now recruiting experienced and compassionate Band 6 Mental Health Practitioners to join Adult Community Mental Health Teams across Manchester, Salford, Trafford, Bolton and Wigan.

This is an exciting time to join us. We're investing significantly in community mental health services, expanding our teams and transforming the way care is delivered – creating larger, safer and more responsive multidisciplinary teams focused on recovery, personalisation and better outcomes.

What you'll be doing

Working within an Adult Community Mental Health Team, you'll provide high-quality, recovery-focused and trauma-informed care to adults experiencing serious mental health difficulties.

Your role could include:

  • Carrying out assessments and contributing to clinical formulation
  • Delivering therapeutic interventions and psychosocial support
  • Acting as a Named Worker and undertaking care coordination where required
  • Managing an agreed caseload
  • Supporting risk management, safeguarding and crisis planning
  • Working closely with carers, GPs, local authorities, VCSE organisations and other services
  • Helping people achieve their recovery goals and progress through their care pathway
  • Working flexibly across the CMHT pathway, responding to changing service needs

There will also be opportunities to gain experience across different areas of the pathway. Depending on service needs, you may initially focus on assessment and formulation, ongoing treatment, care coordination, complex case management or assertive outreach – giving you the chance to broaden your skills and experience.

You will need full registration for the area you work, eg Nurse NMC PIN, Social Work, Social Work England reg.
