Locum Nurse – Key Job Duties

• Conduct initial and 6-monthly health assessments for clients.
• Monitor and record BP, pulse, SpO₂, temperature, weight and other relevant vital signs.
• Monitor clients for health, cognitive, behavioural and functional changes.
• Track improvement, decline, falls, nutrition, hydration, mobility and ADL.
• Maintain and update individual care plans and health records.
• Identify early signs of deterioration and escalate appropriately to the Centre Manager, family or medical team.
• Review and follow up on hospitalisations, medical appointments and changes in medication/health conditions.
• Work closely with physiotherapist, dietitian and care coordinators to monitor client progress.
• Provide basic clinical guidance and training to care staff.
• Ensure accurate and timely clinical documentation and reporting.
