Job Responsibilities:
- Review written/electronic order form (medication chart, medication record or prescription) to ensure drug prescribed is appropriate.
- Perform medication reconciliation.
- Provide drug information service to health care professionals.
- Involvement in clinical services such as attending multidisciplinary rounds and running ambulatory services. Dispenses and counsel patient on medication prescribed by doctor.
- Assigns, oversees and closely review the work of pharmacy assistants and technicians.
- Training pharmacists to be advance generalists specialists. Craft out necessary learning objectives, curriculum, materials.
Requirements:
- At least 5 years' relevant experience as Clinical Pharmacist
- PharmD or equivalent degree recognized by the Singapore Pharmacy Council
- Experience in training and development of clinical curriculum/training programs
- Strong leadership and communication skills
