Participant Care Associate, CNA
Position: Participant Care Associate (CNA) Client: PACE at Hudson Headwaters Location: Glens Falls, NY Target Areas: Queensbury, South Glens Falls, Hudson Falls, Fort Edward, Lake George, Saratoga Springs, Ballston Spa, Wilton, Moreau, Gansevoort, Corinth, Warrensburg, Granville, Whitehall, Schuylerville, Greenwich, Argyle, Salem Pay: $22.50/hour Schedule: Full-Time; Monday–Friday; 8:00 AM - 4:30 PM; No weekends required About the Role: Provide direct personal care and support to older adults enrolled in a Program of All-Inclusive Care for the Elderly (PACE), helping maintain safety, dignity, independence, and quality of life while working as part of an interdisciplinary healthcare team. Key Responsibilities:
- Provide hands-on personal care (bathing, grooming, dressing, toileting)
- Assist with feeding and meal support
- Assist with transfers and ambulation
- Monitor participant well-being and report concerns
- Complete participant care documentation
- Support participant independence and safety
- Follow individualized care plans and collaborate with interdisciplinary team members
- Maintain participant dignity and comfort
Required Qualifications:
- Active New York State CNA Certification
- High School Diploma or GED
- Ability to provide hands-on personal care and work with elderly and frail populations
- Experience or comfort assisting with bathing, grooming, dressing, toileting, feeding, transfers, ambulation
- Ability to work Monday–Friday, 8:00 AM - 4:30 PM
- Ability to lift up to 50 lbs occasionally
- Reliable transportation
- Ability to complete medical clearance and background checks
- Strong communication and documentation skills
- Ability to work within an interdisciplinary healthcare team
