Social Worker – Acute Hospital Discharge Connect Exec are currently recruiting for six experienced Social Workers to join a City Council's Adult Social Care service, supporting the Acute Hospital Discharge Team.

This is an excellent opportunity for experienced Adult Social Workers who have a background in hospital discharge, community social care, assessments and care planning and are looking to take on a rewarding locum position.

The successful candidates will work as part of a team supporting adults to achieve safe and timely discharge from hospital , completing assessments and putting appropriate care and support arrangements in place.

The Role

As a Social Worker within the Acute Hospital Discharge Team, you will be responsible for completing strengths-based, proportionate and person-centred assessments and care planning for adults with care and support needs.

You will manage a varied caseload, including cases involving more complex needs and higher levels of risk, while working alongside adults, their families, carers and wider professional networks to achieve positive outcomes.

Key Responsibilities

• Complete strengths-based, proportionate and person-centred assessments for adults.
• Assess care and support needs and develop effective Care and Support Plans with measurable outcomes.
• Support adults to achieve safe and timely discharge from hospital.
• Manage a caseload ranging from straightforward to complex and higher-risk cases.
• Undertake reviews and reassessments in line with legislation and guidance.
• Work directly with adults, families, carers and informal support networks.
• Promote independence, community inclusion, self-help, early intervention and prevention.
• Identify appropriate services, support and resources to meet assessed needs.
• Undertake safeguarding work, including participating in investigations and strategy meetings where required.
• Work collaboratively with health professionals, colleagues and external agencies.
• Maintain accurate case records and ensure the adult's voice and decision-making rationale are clearly recorded.
• Prepare and present statutory and non-statutory reports.
• Provide information, advice and guidance to adults, families and carers.
• Work within the framework of the Care Act 2014 and relevant legislation and guidance.

About You

We are looking for Social Workers who have:

• A recognised Social Work qualification.
• Current Social Work England registration .
• Experience working within Adult Social Care .
• Experience completing assessments, reviews and care planning.
• Experience supporting adults with care and support needs.
• Ideally, previous experience within hospital discharge, community discharge or an integrated health and social care environment .
• Experience managing a varied caseload, including complex and higher-risk cases.
• Knowledge and understanding of safeguarding.
• Knowledge of the Care Act 2014 and relevant adult social care legislation.
• Excellent communication and relationship-building skills.
• The ability to work effectively with multidisciplinary and multi-agency teams.
• Strong recording, report-writing and organisational skills.
