A leading healthcare and eldercare organisation is seeking a compassionate Social Work Associate to join its growing team.

Your tasks

Job Responsibilities

• Manage case referrals
• Consult with the centre lead to conduct assessment, planning and intervention for cases
• Monitor the delivery of coordinated services
• Adopt a suitable case management approach when working with clients
• Refer and coordinate care services to ensure clients are adequately supported
• Keep abreast of, and provide information on, the various assistance schemes and care options available
• Assess family financial status
• Manage the administration of grants, subsidies and support services where relevant
• Ensure all MOH service requirements on financial and quality aspects are adhered to

Requirements

Job Requirements

• Bachelor’s Degree, preferably in Social Work
• Degrees in Counselling, Gerontology, Psychology, Sociology or related disciplines are also welcome
• Min. 1 year of experience in social work, preferably in community or healthcare settings
• Strong communication and stakeholder management skills
• Proficient in Google Suite applications

What we offer

• Salary: Up to S$3300 (commensurate with experience)
• 5 Day Work Week: Mondays –Fridays
• Location: South Central and West
• Employment Type: Full-Time, Permanent
