Our client, a leader in the provision of community healthcare services is seeking enthusiastic and experienced candidates to join their clinics as a Senior/Staff Nurse (Chronic Disease Management).

Job Responsibilities:

• Provide holistic, evidence-based nursing care for patients with chronic conditions.
• Develop personalised care plans and ensure safe medication administration and accurate documentation.
• Educate patients on chronic disease management, healthy lifestyles, medication adherence and self-monitoring.
• Conduct psychosocial and functional assessments and support behaviour change.
• Coordinate care with multidisciplinary teams and community partners to ensure continuity of care.
• Monitor patient progress, outcomes and follow-up, including recalling defaulters.
• Support the development and implementation of chronic care programmes, clinical protocols and patient education materials.

Requirements

• Post-basic qualification in Diabetes Management and Education / Gerontology / Chronic Disease Management. Other related post-basic qualifications may be considered.
• Diploma/ Bachelors Degree in Nursing
• Experience in nursing and managing patients with chronic conditions is preferred
• Registered Nurse with Singapore Nursing Board
