Your tasks
- Create personalised care plans tailor to the needs of resident
- implement care plans and regularly record residents' progress
- Design and organise programs and activities to address residents' care requirements
- Prepare and submit review reports and monthly interview records every 6 month
- Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents
Requirements
Job Requirement
- Min Diploma/Degree in Social Work/Psychology/Counselling
- At least 1 years relevant healthcare industry experience
- Good interpersonal and problem-solving skills
- Proficient in English and able to converse in dialects or other languages
- Office Hours: 5 days work week
- Enjoys in a Geriatric Care setting
