Medical Social Worker | Geriatric Care

Job Description :

Create personalised care plans tailor to the needs of resident

implement care plans and regularly record residents' progress

Design and organise programs and activities to address residents' care requirements

Prepare and submit review reports and monthly interview records every 6 month

Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents

Job Requirement :

Min Diploma/Degree in Social Work/Psychology/Counselling

At least 1 years relevant healthcare industry experience

Good interpersonal and problem-solving skills

Proficient in English and able to converse in dialects or other languages

Office Hours: 5 days work week

Enjoys in a Geriatric Care setting
