Job Description : Create personalised care plans tailored to the needs of the resident Implement care plans and regularly record residents' progress Design and organise programs and activities to address residents' care requirements Prepare and submit review reports and monthly interview records every 6 month Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents Job Requirement : Min Diploma in Social Work/Psychology At least 2 year of relevant healthcare industry experience Working Hours: Mondays to Fridays, shift work with DO Check out www.abercare.sg for more details
