Position Overview

The Family Nurse Practitioner provides comprehensive primary care services; focusing on prevention, diagnosis, and management of acute and chronic conditions.

This role partners with physicians and interdisciplinary teams to deliver high-quality, coordinated care; supporting organizational goals related to access, quality, and patient experience.

What You’ll Do

Clinical Care & Patient Services

Conduct patient assessments, physical exams, and diagnostic evaluations

Diagnose and manage acute and chronic conditions across the lifespan

Develop and implement individualized treatment plans

Prescribe medications and perform procedures within scope of practice

Provide preventive care, screenings, and health maintenance services

Patient Education & Care Coordination

Educate patients and families on disease management, prevention, and wellness

Collaborate with care teams to ensure coordinated, patient-centered care

Support transitions of care and appropriate referrals within and outside the network

Quality & Population Health

Participate in quality improvement initiatives and care gap closure efforts

Support population health strategies and value-based care programs

Monitor patient outcomes and contribute to continuous improvement efforts

Documentation & Compliance

Maintain accurate and timely documentation within the Electronic Health Record (EHR)

Ensure compliance with HIPAA, regulatory, and organizational standards

Support coding, documentation accuracy, and billing requirements

What You Bring

Required

Master’s or Doctoral degree in Nursing from an accredited program

Current, unrestricted Indiana Nurse Practitioner license

Board certification as a Family Nurse Practitioner (FNP)

Prescriptive authority and DEA licensure (or ability to obtain)

Strong clinical assessment, diagnostic, and decision-making skills

Excellent communication and collaboration skills

Preferred

Experience in primary care, community health, or FQHC settings

Experience working with diverse or underserved populations

Familiarity with EHR systems such as Epic

Experience in team-based or integrated care environments
