Liquid Personnel is recruiting an Adults Social Worker to join its client’s Rapid Discharge to Assess Team based in Trafford.
What will your responsibilities be?
Timely Hospital Discharge Planning: Work closely with health professionals to assess patients' needs and coordinate safe, efficient discharge plans that avoid unnecessary hospital stays.
Holistic Needs Assessment: Conduct strength-based assessments in community settings post-discharge to determine ongoing care and support requirements, ensuring person-centred outcomes.
Multi-Agency Collaboration: Liaise with families, care providers, and partner agencies to implement tailored support packages and facilitate smooth transitions from hospital to home or interim care.

Qualifications & Experience:
To be successful in this role you must have:
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
