An exciting job role has recently become available for an Adult Social Worker. They are looking for a new member to join their Adult’s Review Team based in West London.
What will your responsibilities be?
In this role, you will be undertaking assessments and reviews under the Care Act 2014, holding a caseload, and supporting the duty function. You will be responsible for evaluating the needs of individuals, ensuring they receive appropriate care and support. Your duties include conducting thorough assessments, developing care plans, and regularly reviewing the progress and effectiveness of these plans. You will also collaborate with other professionals and agencies to provide comprehensive support to those in need. Your role is crucial in promoting the well-being and independence of individuals, making a significant impact on their lives.
About You
To be successful in this role, you will have:
A recognised Social Work qualification
Current Social Work England (SWE) registration
Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
Good knowledge of the Mental Capacity Act and relevant legislation
Strong multi-agency partnership working skills
Excellent communication, organisational, and problem-solving abilities
A compassionate, proactive, and person-centred approach to practice
