Liquid Personnel is seeking an experienced Occupational Therapist to support one of its clients in meeting increased demand following the successful rollout of their Home First contract.
This is a high impact reablement role, focused on enabling individuals to maintain independence and reducing the need for long-term care support.
What will your responsibilities be?
Complete strengths-based assessments in line with Care Act principles
Develop reablement-focused care plans to maximise independence
Provide expert advice on equipment and adaptations
Manage a diverse community caseload, including home visits and hospital settings
Work closely with multi-disciplinary teams to support safe discharges and prevent admissions
Maintain accurate records and contribute to service outcomes

Qualifications & Experience:
Qualified Occupational Therapist with HCPC registration
Minimum 2 years post-qualified experience
Experience within:
Reablement / Community OT
Adults Social Care
Full UK driving licence (essential)
Working Pattern:
37 hours per week
~50% of admin completed in the office (County Hall)
Remaining time split between:
Home working
Community visits
Example structure:
3 days visits
1 day office-based
1 day working from home
4-day working week considered (no less)
