Position Summary
As a Hospice Registered Nurse (RN) , you will provide skilled nursing care to patients with life-limiting illnesses throughout the East Bay , helping patients remain comfortable and maintain dignity during every stage of their hospice journey.
You will perform comprehensive nursing assessments, develop individualized plans of care, manage symptoms, educate families, and collaborate closely with physicians and the interdisciplinary team to ensure exceptional patient-centered care.
Responsibilities
Complete comprehensive nursing assessments for hospice patients.
Develop, implement, and evaluate individualized plans of care.
Provide skilled nursing visits in patients' homes, assisted living communities, board and care homes, and skilled nursing facilities.
Assess and manage pain and symptoms using evidence-based hospice practices.
Administer medications, treatments, and interventions as ordered.
Educate patients and caregivers on disease progression, medications, symptom management, and end-of-life care.
Coordinate care with physicians, social workers, chaplains, home health aides, and other interdisciplinary team members.
Communicate changes in patient condition promptly to the Hospice Clinical Director and attending physician.
Complete accurate and timely documentation using WellSky (Kinnser) .
Participate in Interdisciplinary Group (IDG) meetings and quality improvement initiatives.
Maintain compliance with Medicare Conditions of Participation, California regulations, and agency policies.
Qualifications
Current California Registered Nurse (RN) license in good standing.
Minimum one year of RN experience required.
Previous Hospice or Home Health experience strongly preferred.
Strong clinical assessment and critical thinking skills.
Compassionate, dependable, and patient-focused.
Excellent communication and organizational skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.
Current California driver's license, reliable transportation, and proof of auto insurance.
Current CPR certification.
