The Role

Vivid Healthcare are currently recruiting for an experienced Band 6 SPA Triage Clinician to join the Single Point of Access (SPA) service at Sirona care & health .

This is an exciting opportunity for a registered healthcare professional with strong clinical assessment and triage experience to work within a multidisciplinary team, providing telephone-based clinical triage across community health services.

The SPA is responsible for assessing referrals and visit requests for adult patients and ensuring they are directed to the most appropriate service, helping people receive care within the community where appropriate and supporting early discharge from hospital.

Key Responsibilities

  • Provide person-centred telephone clinical triage of referrals and visit requests.
  • Assess clinical need and urgency and determine appropriate intervention timeframes.
  • Allocate patients to the most appropriate community service or services.
  • Provide clinical advice, support and signposting where appropriate.
  • Identify when specialist clinical input is required and make appropriate referrals.
  • Work closely with patients, families, carers, GPs, hospitals and other professionals.
  • Identify and escalate adult safeguarding concerns appropriately.
  • Maintain accurate and contemporaneous patient records.
  • Use specialist assessment tools and clinical resources.
  • Promote self-care, independence and improved quality of life.
  • Support quality improvement, research and service development initiatives.
  • Take responsibility for coordinating the SPA when rostered as Clinical Coordinator.
  • Support, supervise and mentor colleagues and students where required.

About You

The successful candidate will be a confident and experienced registered healthcare professional who can demonstrate:

  • Current professional registration with the NMC, HCPC or relevant professional body .
  • Experience working across acute and community settings .
  • Experience of working across organisational boundaries within health, social care or mental health.
  • Strong clinical assessment, analytical and problem-solving skills.
  • Excellent verbal and written communication skills, including effective clinical triage.
  • Ability to build strong working relationships with patients and professionals.
  • Good IT skills, including Microsoft Office.
  • Experience working with external agencies and organisations.
  • Knowledge of person-centred assessment and clinical governance.
  • Understanding of relevant legislation including confidentiality, data protection and the Mental Capacity Act.

Clinical Experience

Candidates should have knowledge of a range of nursing procedures, including:

  • Wound care
  • Catheter care
  • Medicines administration
  • Cannulation
  • IV antibiotics
  • Subcutaneous infusions
  • Sepsis management
  • NEWS escalation

Working Environment

The SPA operates a 7-day service , with shifts covering 07:30–20:00 , including weekends and bank holidays.

The service operates virtually across Bristol, North Somerset and South Gloucestershire , with bases in Withywood, Warmley and Clevedon. Some homeworking may be available following successful completion of probation.

Occasional travel to other bases or locations may be required for training, meetings and service requirements.
