Job Description : Create personalised care plans tailored to the needs of the resident Implement care plans and regularly record residents' progress Design and organise programs and activities to address residents' care requirements Prepare and submit review reports and monthly interview records every 6 month Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents Job Requirement : Min Diploma/Degree in Social Work/Psychology/Counselling At least 1 year of relevant healthcare industry experience Good interpersonal and problem-solving skills Proficient in English and able to converse in dialects or other languages Office Hours: 5 days work week Check out www.abercare.sg for more details
